1. Call us at 858-376-1440 to set up an appointment.
2. If there is enough time between your call and the appointment, we can mail the "New Patient Starter Kit" to you so you can fill out the following forms in advance:
   • New patient check-off list
   • Medical history
   • HIPAA (privacy policy) form
   • Dental history
   • Registration
   • Financial policy for insurance policyholders
   • Dental claim form
   • Emergency form
   • Informed Consent Form
   Or, you can fill them out when you arrive at our office (please arrive 15 minutes in advance to allow time to fill them out).
3. Show up at the appointed time.
